Kidney Infection: Pyelonephritis

Pyelonephritis, Pyelitis, and Urosepsis refers to infection in the pelvis of the kidney. Sepsis is the natural inflammatory response to an infection.  The symptoms of this very painful condition includes pain while urinating called dysuria , pain in the abdominal region  and the bladder area along with the region around the affected kidney becomes tender. Systemic symptoms like fever, headache, violent shivering, vomiting and in extreme cases delirium may occur.

Diagnosis of this disease is fairly easy and can be confirmed on conducting a diagnostic test. Patients who show typical symptoms take a urine dip stick test. Presence of nitrite and white blood cells can definitely confirm pyelonephritis. This is sufficient enough for starting treatment. Diagnosis with the aid of urine culture can confirm the disease. If there is a lack of symptoms then one has to go in for blood culture test.  If the signs include large amount of blood in urine and colicky pain it could be signs of kidney stone.

In such cases X-Rays of the kidney and the urinary bladder must be taken to assess the disease. X-ray can reveal kidney stones as it is radio-opaque. If the disease occurs again and again then it could be a case of flowing back of the urine from the bladder. It could be also a case of polycystic kidney disease. Such situations might require the need for taking ultrasound scan of the kidneys or cystourethrography. Infection of the area could also be a result of cancer and should not be taken lightly. Biopsy might be required to determine if it is due to cancer or not.

Polycystic kidney disease  is a genetic disorder of the kidney , the feature of this disease is the formation of multiple cysts found in both the kidneys.  Major discomforts associated with other urinary infections  also forms part of PKD.  The sensitivity of renal ultrasonography for the detection of ADPKD is 100% for subjects 30 years or older with a positive family history. Diagnostic criteria require two or more cysts in one kidney and at least one cyst in the contralateral kidney in young subjects, but four or more in subjects older than 60 years because of the increased frequency of benign simple cysts. Most often, the diagnosis is made from a positive family history and imaging studies showing large kidneys with multiple bilateral cysts and possibly liver cysts. Before the age of 30 years, CT scan or T2-weighted MRI is more sensitive for detecting presymptomatic disease because the sensitivity of ultrasound falls to 95% for ADPKD type 1 and <70% for ADPKD type 2.Genetic counseling is essential for those being screened. It is recommended that screening for asymptomatic intracranial aneurysms should be restricted to patients with a personal or family history of [[intracranial hemorrhage. Intervention should be limited to aneurysms larger than 10 mm. Someone with this disease has a 5% chance of getting brain aneurysms.

Treatment

At present, treatment is largely supportive, as there is no single therapy that has been shown to prevent the decline in kidney function. Hypertension control with a target blood pressure of 130/85 or less is recommended.Lower levels have been reported to slow the rate of loss of kidney function. A multidrug approach that includes agents to inhibit the renin-angiotensin system is frequently required. There is no compelling evidence to recommend a low-protein diet, especially in patients with advanced kidney dysfunction where optimizing nutritional status is important. Lipid-soluble antimicrobials, such as trimethoprim-sulfamethoxazole and quinolone antibiotics that have good tissue permeation, are the preferred therapy for infected kidney cysts. Pain management occasionally requires cyst drainage by percutaneous aspiration, sclerotherapy with alcohol or, rarely, surgical drainage. Patients with ADPKD appear to have a survival advantage on either peritoneal or hemodialysis compared to patients with other causes of ESRD. Those undergoing kidney transplantation may require bilateral nephrectomy if the kidneys are massively enlarged or have been the site of infected cysts. Horshoe Kidney

Horse shoe kidney is a condition which occurs while a foetus and is also known as renal fusion . In this condition  both the kidneys fuse together to form a horse shoe shape.  There are two groups of kidney fusion one is  horse shoe kidney and the other is crossed fussed ectopia. Persons suffering from this disease may suffer from nausea ,abdominal discomfort,kidney stones and other  infections. This condition can lead to kidney obstruction which may block the ureter and causes the dilation of kidneys.  Other conditions include infection of kidneys, kidney stones ,kidney cancer, etc. People with turners syndrome is found to have horseshoe kidneys and the rate is as high as 15% in females. Only the symptoms of this disease can be treated not the disease itself. It is also associated with trisomy 18.

Hard, crystalline mineral material formed within the kidney or urinary tract is kidney stone. They cause blood in the urine and severe pain in the abdomen, flank, or groin. Kidney stones usually form when there is a decrease in urine volume and excess of stone-forming substances in the urine. They are also referred as renal calculi. One in every 20 people develops a kidney stone at some point in their life. The condition of having kidney stones is termed nephrolithiasis. Having stones at any location in the urinary tract is referred to as urolithiasis. The most common type of kidney stone contains calcium in combination with either oxalate or phosphate. Other chemical compounds that can form stones in the urinary tract include uric acid and the amino acid cystine.

Stones can also be formed due to the obstruction in urine flow. Kidney stones can also result from infection in the urinary tract; these are known as struvite or infection stones. Kidney stones begin at the age of 40s and continue to develop until 70s. People who have already had more than one kidney stone are prone to develop more stones. A family history of kidney stones is also a risk factor for developing kidney stones. Dehydration from reduced fluid intake or strenuous exercise without adequate fluid replacement increases the risk of kidney stones. Other conditions associated with an increased risk of kidney stones include hyperparathyroidism, kidney diseases such as renal tubular acidosis, and some inherited metabolic conditions including cystinuria and hyperoxaluria. Chronic diseases such as diabetes and high blood pressure (hypertension) are also associated with an increased risk of developing kidney stones.People with inflammatory bowel disease or who have had an intestinal bypass or ostomy surgery are also more likely to develop kidney stones. Some medications also raise the risk of kidney stones. These medications include some diuretics, calcium-containing antacids, and the protease inhibitor indinavir (Crixivan), a drug used to treat HIV infection.

Cancer of the Bladder

Bladder cancer is a cancer that forms in tissues of the bladder, also refers to several types of malignant growths of urinary bladder. Bladder cancer can be transitiona cell carcinomas (cancer that begins in cells that normally make up the inner lining of the bladder), squamous cell carcinoma (cancer that begins in thin, flat cells) and adenocarcinoma (cancer that begins in cells that make and release mucus and other fluids). As a result of chronic irritation and inflammation, the cells forming squamous cell carcinoma and adenocarcinoma develop in the inner lining of the bladder. Abnormal cells continue to multiple without any control in the bladder.

Bladder cancer causes blood in the urine which may be visible to the naked eye or detectable by microscope. Other possible symptoms include pain during urination, frequent urination (Polyuria) or feeling the need to urinate without results, pain when you urinate and low back pain. Major risk factor for bladder cancer is smoking. Exposure to certain chemicals in the workplace is another. People with a family history of bladder cancer or who are older, white or male have a higher risk. Treatments for bladder cancer include surgery, radiation, chemotherapy and biologic therapy.
